Resumo Serum TSH levels are paradoxically elevated in patients with hypothyroidism due to Sheehan's syndrome (Abucham et al 1997 Clin Endocrimol 47:515-22). To further understand this apparent discrepancy, the biological activity and glycosylation pattern of serum TSH was studied in 5 untreated patients with Sheehan's syndrome and in 6 normal controls. TSH bioassay was based on cAMP generation, measured by RIA, in a cell culture system of CHO cells transfected with recombinat human TSH receptor. The oligosaccharide branching of TSH was studied by concanavalin-A (Con-A) lectin affinity chromatography. TSH bioactivity (B/I) was expressed as the ratio between biological and immunofluorimetric assays. Statistical analyses were done by Student's t test or Mann-Whitney U-test. Results are mean ñ SD. Serum FT4 levels were lower in patients than in controls (0.36ñ0.03 µg/dL vs 1.13ñ0.17 µg/dL, respectively, P=0.0001). Circulating immunoreactive TSH was higher in patients with Sheehan's syndrome as compared to controls (4.38ñ1.99 mU/L vs 2.2ñ1.04 mU/L, respectively, P=0.08). In contrast, TSH B/I was significantly decreased in Sheehan's patients as compared to control (0.75ñ0.3 vs 1.9ñ0.8, respectively, P<0.02).
